Largest open GHL opportunity ($10k). Transform Partners consultant supporting a CA endocrinology practice running the My Diabetes Tutor (MDT) telehealth line, now expanding nationally. Opportunity is custom API / bulk ingestion work into MDT's Charm instance. 2026-08-25: Otter pt.2 (8/25 call): Four open technical questions — (1) does Charm support bulk population of patient charts via custom async API; (2) best input format: direct JSON from X Cures vs CCDA package via CareMesh; (3) must supporting docs (CCDA/PDF/images) be stored in Charm for auditability, or parsed metrics only; (4) transaction cost / API-call volume economics at cohort scale. Pilot: Chicago FQHCs on Epic to reduce variability, MDT stays on Charm = two-EHR integration. Volumes 9,000 + 7,000 already referred, manual charting impractical. Commercial: broader proposal in final edit, may go to MDT and Aetna in ~1–1.5 weeks; Michael expects Aetna funds most of the larger effort but wants all partners cost-conscious; Charm component deliberately deferred from the main proposal until data design is clear. 2026-08-25: CORRECTION (logged by dan 2026-08-25): the Otter summary call took place Tue 2026-08-18, NOT 8/25. It is the same 8/18 12:30 PDT consultation already recorded in the brain — there was no 8/25 call. The CRM call record dated 2026-08-25 is that 8/18 call; treat its true date as 2026-08-18. Consequence: Michael's "answer by end of week" ask referred to w/c 8/17, so it is already ~1 week overdue, and the proposal ship window (~1–1.5 weeks from 8/18) lands roughly 8/25–8/29 — i.e. now. 2026-08-25: Email added from GHL: Michael@transformpartners.health. 8/24 follow-up call held — Dan + Hugh + Conner met with Michael (the Hugh/Conner handoff account, first joint call). Technical scoping call (Otter summary). Question on the table: can Charm accept bulk asynchronous ingestion of curated patient records for My Diabetes Tutor? Context: Aetna Better Health of Illinois has already sent MDT Medicaid diabetic cohorts of 9,000 and 7,000. Direct-to-patient outreach underperformed, so the plan is to engage PCPs/care teams inside FQHC workflows. Data flow: Aetna refers -> X Cures pulls records via TEFCA/mainly Carequality -> X Cures AI parses down to diabetic-care data tied to HEDIS measures (plus new mental health and SDOH elements) -> JSON payload to CareMesh -> CareMesh inserts patient-specific content into secure provider messaging templates. Unresolved leg: how the curated record (and possibly CCDAs, PDFs, images) gets into MDT's Charm instance. Open questions: does the payload come direct from X Cures as JSON or via CareMesh as CCDA; does MDT need source documents stored in-chart for auditability/continuity; lowest-cost approach that still places a complete record. Constraint: manual chart creation is fine at ~25 referrals, breaks at payer cohorts in the thousands. Likely pilot narrows variables to Chicago FQHCs on Epic while MDT stays on Charm. Michael deliberately left the Charm solution out of the broader proposal until record design is clearer, and wants a fast conceptual feasibility answer for MDT and Aetna before detailed pricing. Open items carried in: four technical questions (bulk async chart write, JSON vs CCDA input, whether source docs must be stored in-chart, per-transaction API economics at 16k-patient scale), the HEDIS/data-mapping document Michael owes, and whether the broader MDT/Aetna proposal has shipped with the Charm section still open.
